A 54-year-old male smoker with stage IV adenocarcinoma located in the left lower lobe. Pre-treatment chest CT (a) demonstrated ground-glass opacity, reticulation, traction bronchiectasis, and honeycombing predominantly located in bilateral lower lobes, in accordance with subpleural fibrotic ILA. The tumor was located in ILAs and presented the lobulation sign (a, b). The patient underwent fine needle aspiration biopsy, with a pathological diagnosis of adenocarcinoma without EGFR mutation. He was died 300 days after the diagnosis of lung cancer. CT, computed tomography
